We have been advised by NHS England that we can now take referrals from health and social care professionals for the following specific patient groups:
- Wheelchair user who cannot transfer without a hoist
- Bariatric patient over 25 stones
- Domiciliary patients who are housebound
- Patients living in a secure mental health facility
- Patients under the care of homeless/drug & alcohol services
- Patients who are referred by a consultant psychiatrist due to significant mental health issues.
Additional criteria for patients resident in Suffolk (excluding Waveney):
- Patient diagnosed with autism who is unable receive care in general dental practice
- Patient with a diagnosed learning disability who is unable receive care in general dental practice
- Patient diagnosed with dementia (any stage).
