Health and Social Care Referrals

We have been advised by NHS England that we can now take referrals from health and social care professionals for the following specific patient groups:

  • Wheelchair user who cannot transfer without a hoist
  • Bariatric patient over 25 stones
  • Domiciliary patients who are housebound
  • Patients living in a secure mental health facility
  • Patients under the care of homeless/drug & alcohol services
  • Patients who are referred by a consultant psychiatrist due to significant mental health issues.

Additional criteria for patients resident in Suffolk (excluding Waveney):

  • Patient diagnosed with autism who is unable receive care in general dental practice
  • Patient with a diagnosed learning disability who is unable receive care in general dental practice
  • Patient diagnosed with dementia (any stage).
